MPXV is an enveloped double-stranded DNA virus that is a member of the genus OPXV of the Poxviridae family. MPX is a zoonotic disease caused by the MPXV that behaves similarly to smallpox. Headache, fever, muscle aches, swollen lymph nodes, and rash are common symptoms of MPX infection. The clinical differentiation of the disease from smallpox and chickenpox is difficult. Laboratory diagnosis is a major component of disease identification and surveillance, and multiple different strategies are required for a more precise and rapid diagnosis.
Over the past decades, various methods have been used for MPX detection. Methods for laboratory evaluation of suspected cases of MPX include electron microscopy (EM), PCR testing, IHC, culture of rash sample material, and positive OPXV-specific serology to detect antibodies. Each approach has its pros and cons. For example, PCR detection is virus-specific. Therefore, it is beneficial to distinguish between different OPXV species and provide direct evidence for the presence of the acute virus. Notably, serological testing, especially IgM testing, plays a critical role in assessing exposure and immunity to OPXV.
IgM antibodies are the first antibodies that appear in MPX infection, so they can be detected at the onset of the disease. Anti-OPXV IgM can be used to detect the presence of OPXV antibodies. The development of an OPXV-specific IgM assay allows the detection of recent exposure to OPXV by measuring IgM-class antibodies, indicating an acute-phase immune induction. Furthermore, serology is superior to PCR in terms of diagnostic power due to the stability and duration of antibody responses. It is worth noting that the detection of OPXV-specific IgM using enzyme-linked immunosorbent assay (ELISA) provides diagnostic support for the evaluation of OPXV infection.
